Complete Your Fast Track Health History Questionnaire Online

Effective March 14, 2022, masks are optional except where required by blood drive host preferences or local regulations. Vitalant will continue to support those who need or prefer to wear a mask and will continue to have complimentary masks available.

It takes only about 10 minutes to answer the yes/no health history questions. Fast Track will save you about 20 minutes at the blood drive or blood center.  Watch our short video on how to complete your Fast Track questionnaire.  

 

Important Note: Please read the following documents prior to beginning the Fast Track Health History questionnaire. The questionnaire will refer to these documents so it is important you read them in their entirety. Please be sure to complete the questionnaire on the same day as your donation and be sure to use your name exactly as it appears on your legal ID. After you complete the Fast Track questionnaire, you will receive a QR code; please bring the QR code with you to your appointment.
