Redeem Your 2023 Waste Management Phoenix Open Voucher

Thank you for donating blood Jan. 1 - 13!  Complete the fields below by Friday, Jan. 20, to redeem your voucher for a one-day general admission ticket to the 2023 Waste Management Phoenix Open, courtesy of The Thunderbirds!  

Your Vitalant voucher is not redeemable at the tournament.  You must complete this form to redeem your voucher for a ticket, which will be mailed to you through the U.S. postal service.  All fields must be completed by Friday, Jan. 20, at 10 p.m.  Tickets will be mailed on a first-come/first-served basis.  Your voucher/ticket is non-transferable and cannot be redeemed or sold for cash.  One voucher/ticket per donation.  For questions, please call Vitalant at 480.933.7229.
