Redeem Your 2026 WM Phoenix Open Ticket Voucher

Thank you for donating blood! Complete the fields below to redeem your voucher for one ticket to the 2026 WM Phoenix Open.

Your Vitalant voucher is not redeemable at the WM Phoenix Open box office, nor does it serve as admission to the tournament. You must complete the below form to receive your electronic ticket.   

REGISTRATION ON THE AXS TICKET SITE IS REQUIRED TO ACCESS YOUR ELECTRONIC TICKET. YOUR AXS EMAIL MUST MATCH THE EMAIL YOU PROVIDE TO REDEEM TICKETS WITH VITALANT.

• Please designate your ticket date first and second choice.
• Supplies are limited. Electronic tickets will be distributed on a first-come, first-served basis while supplies last.
• Your electronic ticket is non-transferable and cannot be redeemed or sold for cash.

You will have your choice of the following days:

• Wednesday, Feb. 4
• Thursday, Feb. 5
• Sunday, Feb. 8

Your Vitalant voucher must be refeemed no later than 10 PM Friday, Jan. 16, 2026. Electronic tickets will be emailed by Jan. 28, on a first-come, first-served basis, while supplies last.
